Personal Shopping does not work this way.
If you want to be a personal shopper, you sign up with agencies who contact you with various assignments and they pay you per assignment. You do not become their employee – you are an independent contractor and if you earn over a certain amount of money in a given year then they send you a 1099 (not a W-2).
If you want to start a personal shopping company and negotiate contracts with various stores/restaurants/etc. then you must get a business license and go through all the requirements of becoming a small business. Then you must solicit people who are willing to take the assignments that the stores offer to you. Those people (personal shoppers) send you reports which you format and send to the store. You pay the personal shopper for their completed assignments and you send them a 1099. The store pays you per assignment.
